What is a chronic disease? (As defined by Medicare)

A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. There is no list of eligible conditions; however, the CDM items are designed for patients who require a structured approach, including those requiring ongoing care from a multidisciplinary team.

What programs are available at Active Recovery?

Everyone attending active recovery will have an initial assessment with either a Physiotherapist or Exercise Physiologist.  We will talk to you about your condition; discuss what we can offer you and the various ways that you can achieve your goals.

Supervised individual sessions can be land based or aquatic based and focus on exercise and activity in our customised exercise space or hydrotherapy pool. 

Group activity sessions:  Small groups of 4-6 people, exercise circuit (land based) or hydrotherapy group class.

Prescription of a home exercise / activity program

Prescription of an exercise program to undertake at your own gym.

Referral to community programs, for example, walking for pleasure.

Who is eligible for referral?

Anyone can attend active recovery for a chronic condition. You may be eligible for referral under a chronic disease management plan, which enables you to access a rebate for up to 5 sessions from Medicare.  Please discuss this with your General Practitioner.

You may also attend privately. You do not require a referral. You may be eligible for a rebate under private health insurance, if you have the appropriate cover.
